如何填写CB测试证书申请书：
1. 用英文填写此申请书；
2. 中国电工产品认证委员会的英文名称：China Commission for Conformity
Certification of Electrical Equipment填写在申请书首页右上部的四个小黑点之间；
3. 申请日期要填写完全并清楚；
4. 如果申请人与制造厂是同一单位就分别在栏内填写相同的名称和地址；
5. 整机中使用的已获得CB测试证书的元件栏内，如有，应填元件名称及其CB证书号；
6. 法人代表签字，法人代表的名字用标准字体打印出汉语拼音，同时其本人亲自签字。

The CB Test Certificate is to
cove the following kind of equipment (such as
cable, rewirable or nonrewirable plug, switch,
washing machine, etc.)
CB测试证书包括下述种类的产品（如电缆、重接或不可重接的插头、开关、洗衣机等）
Trade mark or trade marks of the
equipment 产品商标
Description of equipment: In general: type designation,
catalogue number, etc. For cords and cables:
code, designations according to the relevant
standard and number of cores and cross-sectional
areas, etc. For other equipment: the information
specified in the relevant standard.
产品说明：一般包括型号、规格等，对于电线电缆包括相应标准的型号、芯线数和截面积等，对于其它产品应包括相应标准中规定的信息。
Reference to all CB Test Certificates
which have been issued for components used in
the appliance, such as fuses, switches, appliance
couplers, flexible supply cords and cables,
plugs, etc. In case of retesting: Reference
to previous application.
整机中使用的下述元件如：熔断器、开关、电器耦合器、电源线和软电缆线、插头等，如已获得CB测试证书可提供作为参照。如需复测，参考以前的申请。
Other information of value to
the NCBs (such as catalogues, price lists, advertising
material, informatory pamphlets, lists of components
or spare parts, instructions for maintenance
and repair, photographs, drawings, circuit diagrams)
is enclosed (preferably in quadruplicate) and

We are aware of the fact that
when Procedure I is applied the present application
may be cancelled by the First Testing Station,
if we have not handed in sufficient test samples
to the two Testing Stations before 6 months
after receipt of the requests of the Testing

the CB Scheme.
